problem with disappearing toolbars on Mac

One of my customers has a problem where the toolbar disappears and
reappears each time he clicks in the main window of my application.
The problem only occurs if he uses version of my software built on Qt4
(i.e. not Qt3 versions). He is using MacOSX 10.4.6.
Only one customer has reported this so far and I can't reproduce the
behaviour here. I suspect it may be a problem with how his Mac is
configured or a problem with a third party app.
Has anyone seen anything like this?
Can anyone else reproduce the problem?
